Ideal for those with 2 chronic diseases (high cholesterol and hypertension for example). Includes 6 visits per years and 2 acute/sick visits per year. Extra visits billed at $60/each.

Appropriate for those with 3 chronic diseases. Includes 8 visits per year and 2 acute/sick visits per year. Additional visits billed at $60/each.

Notes on Memberships

Memberships are based on an annual contract, and billed monthly. Every membership includes the following:

• Annual labs included in membership

• Access to advanced lab tests with advanced diagnostic capabilities

• Unlimited medical consults/check-ins (text or phone)

• Nutrition consultations

• Ongoing support

• Memberships can be cancelled with 30 days notice at any time.

• Partial refunds on any pre-paid packages are decided on the case-by-case basis.

Functional Medicine Starter Program

This is a three month intensive membership that includes advanced lab interpretations, root cause care plans, nutritional/lifestyle protocols, and weekly check-ins as well as scheduled visits. Includes 1 visit per month. Additional visits billed at $60/each
